Either ways, sometimes when an unopened pack of condom is teared open, it tears open diagonally and into the condom and might damage it - you wouldn't notice this if you do it in the dark or if you're rushing, which was my basis for saying it wasn't on purpose.

That, or your penis is thicker than most, and you just needed a bigger condom.
..or she used an undersized condom, which will break easily - how to tell is if you felt it was very tight after the condom was rolled onto your penis, then yes, you know it was too small.
